If you change your name, address, telephone number, mobile phone number or email address please notify Skene Medical Group as soon as possible. It is important that the contact information we hold for you in your medical records is up to date should we need to communicate with you.
Please remember that if you or your family move outside of the Practice area then you should register with a doctor in your new locality.
You can download the appropriate information form below and hand into the surgery along with proof of change of name or address. If you only wish to change your contact number, please call the surgery.
