Accessing Your Medical Records

Access to Your Medical Recordsyour care data

Patients can ask to access their medical records or have copies of part or the whole records.

Under the Data Protection Act 1998 and the Access to Health Records Act 1988, there is a specific application process that the Practice adheres to.

You can apply in writing to the Practice Manager if you would like access to or copies of your medical records. It is helpful to be as detailed as you possibly can when requesting access to ensure that you receive the information that you require. There is a fee attached to Access to Health Record requests, up to a maximum of £50.

Freedom of Information Act

Information about the General Practioners and the Practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the Practice Manager
