Patient Registration
To register with the Practice you will be required to complete a Registration Form, provide photographic proof of identity and address for each family member or person wishing to register. You can download and complete the forms linked below. You can then bring all of the completed documentation and proof of identity with you to the Practice Reception.

Further Registration Information
We need to ensure that all new patients reside within the Practice boundary area. We have a map in reception where you can check if you live within our Practice boundary. Please ask at Reception if you are uncertain. The Records Administrator will later check your registration application to ensure that you are living in the Practice boundary area before processing your application to register. We provide GP services to the population of Westhill and some of the surrounding area.
Overseas Patients
If you are from overseas, it is important that we establish your residency status within Scotland. You will be asked to produce documentation to support your application to register with the Practice (Passport, Visa, Employment Contract or letter from your employer)
NB: If you move out of the area, you will need to register with another GP Practice. Our staff can give further information.
