Patient Registration

To register with the Practice you will be required to complete a Registration Form, provide photographic proof of identity and address for each family member or person wishing to register. You can download and complete the forms linked below. You can then bring all of the completed documentation and proof of identity with you to the Practice Reception.

Further Registration Information

We need to ensure that all new patients reside within the Practice boundary area. We have a map in reception where you can check if you live within our Practice boundary. Please ask at Reception if you are uncertain. The Records Administrator will later check your registration application to ensure that you are living in the Practice boundary area before processing your application to register. We provide GP services to the population of Westhill and some of the surrounding area.

Overseas Patients

If you are from overseas, it is important that we establish your residency status within Scotland. You will be asked to produce documentation to support your application to register with the Practice (Passport, Visa, Employment Contract or letter from your employer)

NB: If you move out of the area, you will need to register with another GP Practice. Our staff can give further information.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP Practice for 14 days, please complete the temporary registration form following the link below.

You can be registered as a temporary patient for up to three months if you are staying within our Practice boundary area temporarily. This will allow you to be on the our Practice list but still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with Skene Medical Group if you are planning to stay in the area for some time.

To register as a temporary patient simply contact Skene Medical Group on 01224 849400 or download a Form click here Temporary Resident Registration Form and bring this, together with photographic ID to the Practice Reception.

We do not have to accept you as a temporary patient although we do have an obligation to offer emergency treatment if you require it.

You cannot register as a temporary patient at a Practice in the town or area where you are already registered as a permanent patient.

Change of Personal Details

If you change your name, address, telephone number, mobile phone number or email address please notify Skene Medical Group as soon as possible. It is important that the contact information we hold for you in your medical records is up to date should we need to communicate with you.

Please remember that if you or your family move outsie of the Practice area then you should register with a doctor in your new locality.
